New Students to the District » SPMS
If you want to register your child for next school year (2008-2009)
If you are new to the District and need to register your 6th, 7th
or 8th grade student for next school year (2008-2009), use the
following dates to determine your registration dates.
April 14 - June 27: Contact the
Middle School Registrar at 626-441-5830, X3908, to make an
appointment to register your child. You may not register for school
until you reside in South Pasadena and have your appropriate three
proofs of residency. Registration must be done by the parent or legal
guardian of the child. July 1 - August 15: Middle School is closed. No registrations will be processed. Enrollment/Registration Information : Documents required for SPMS registration ADDRESS MUST BE VERIFIED BEFORE REGISTRATION PACKET IS GIVEN (SPMS Office: 8:30am-4 pm) One item from each of the following six (6) sreas must be presented at the time of registration. All documents must be original (no photocopies will be acepted), and must include the name of the parent/guardian and the South Pasadena address. 1. DRIVER'S LICENSE OR LEGAL PHOTO (I.D.) OF PARENT/GUARDIAN **For cases of guardianship, additional information will also be required.** 2. VERIFICATION OF STUDENT BIRTH DATE (Choose one (1) of the documents listed below:) - Birth Certificate - Hospital Certificate - Student Passport 3. STUDENT IMMUNIZATION INFORMATION (Choose one (1) of the documents listed below:) -California Immunization Record --(7th graders must be fully immunized for Hep. B, have 2 MMR vaccines & proof of Varicella vaccine or the disease.) --Out of state and out of country students must have proof of MANTOUX test for TB. - School Records - Doctor Records Three (3) residency verification documents are required for enrollment: 4. CATEGORY A: (Choose one (1) of the documents listed below:) - Deed of Trust - Mortgage Payment Receipt - Rental Agreement (must be within the last year and must list ALL occupants names, and the name, address and telephone number of the landlord) - Property Tax Bill - Closing Escrow Papers 5. CATEGORY B: (Choose one (1) of the documents listed below:) - Gas Bill or connection receipt - Edison Bill or connection receipt - Water Bill or connection receipt - Time Warner Cable Bill or connection receipt - Telephone Bill or connection receipt (If submitting a connection receipt, an actual bill must follow it within one month.) 6. CATEGORY C: (Choose one (1) of the documents listed below:) - Current Car Registration - Car Insurance Statement - Health Insurance Statement - Current Bank Statement - Pay Stub - Current Federal or State Tax Forms ** For a newly established residence, for verification from Category B, a signed, original receipt or statement on letterhead from the utility company will satisfy the requirement, provided the name of the parent/uardian and South Pasadena address were service will be established are listed. |
